This video presents a challenging case of endophthalmitis whereby a patient is seeking a second opinion for their condition. Upon evisceration of the eye and application of elaborated ultrasound biomicroscopy (UBM) the authors discuss how a new diagnosis is established. The video begins with an introduction and overview of the history and signs of the case followed by ultrasound biomicroscopy and ultrasound with techniques described for diagnosis of the case. The final segments include an overview of anterior segment surgical treatment and postoperative outcomes
