Summary Dr. Albert Viau has developed a national physician's assistant program to help solve the problem of rural health service delivery in Republica, a mountainous Central American country. However, his proposed solution has run into opposition from his colleagues and the health establishment. Viau presented a paper outlining the idea in detail to the 32nd National Medical Congress. Many physicians reacted violently against the idea. The case ends with a decision point for Dr. Viau.
